Fax: 864-688-1705 ReferralsAnyone can make a referral to Open Arms Hospice - physicians, nurses, social
workers, family or friends. To qualify for hospice care, the patient's attending
physician and the Hospice Medical Director must certify that the patient has a
life-limiting illness with a life expectancy of six months or less. Our staff
can contact the patient's physician to verify diagnosis and prognosis.
To make a referral, please call us at 864-688-1700 or toll free at 1-866-473-6276. Physicians can fax referrals to 864-688-1705.
